Do heat and air pollution harm 9/11 responders’ health?

The people who worked the rescue, recovery, and cleanup after September 11, 2001, were exposed to toxic chemicals and extreme psychological stress. The researchers matched each home address to yearly fine-particle air pollution (the tiny particles called PM2.5), yearly temperature, and street greenery, a measure of how many trees you can see from the street.
First they looked at each exposure alone, comparing a home at the 25th percentile with one at the 75th. More air pollution went with about 2.3% higher frailty scores, 2.1% higher PTSD scores, and 3.9% higher depression scores. Higher temperature raised all three by less than 1%. More greenery pointed the other way, with about 1.4% lower frailty scores, 1.9% lower PTSD scores, and 3.8% lower depression scores. In every one of these results, the likely range stayed entirely on one side of zero, so chance alone is an unlikely explanation.
Then they treated all three as one combined mix, since heat, pollution, and trees go together in a dense city. Each time all three stepped up together by one decile (a tenth of the way along the ranking from lowest to highest), frailty scores rose 0.56% (very likely between 0.14% and 0.98%) and PTSD scores rose 0.31% (very likely between 0.07% and 0.55%). Temperature supplied 98% of the frailty effect and 73% of the PTSD effect. Greenery still pulled scores down inside the mix, but heat and pollution outweighed it. The combined mix showed no clear link with depression scores.

This was an observational study, so it shows links, not cause and effect. Its strengths are size and length. It drew on more than 300,000 repeated test scores, and about 73% of responders contributed at least four visits. There are real gaps. Greenery was measured once, from 2020 street images, and treated as fixed over the whole study.
