Does menopause cause women’s pulse pressure to rise?

Not the start of it. In the Framingham Heart Study, women’s pulse pressure hit its low point and began to rise in their late 30s, up to two decades before menopause. Men reached the same turning point in their late 40s, about 10 years later.

Pulse pressure is the gap between the top and bottom numbers of a blood pressure reading. A wider gap is a sign that the aorta, the body’s largest blood vessel, is becoming stiffer and less flexible. The rise in women’s pulse pressure after midlife has long been blamed on falling estrogen at menopause. This study tested that idea by comparing when each woman’s pulse pressure turned upward with when her periods stopped.

How pulse pressure changes with age

Pulse pressure does not simply climb through life. Between early adulthood and midlife it usually falls, because the aorta widens and blood flows through it more easily. After midlife the aorta stops widening and its walls stiffen, so pulse pressure rises again. Young women have lower pulse pressure than young men, but that order flips later in life. The flip is sharper in women, who start life with fewer elastic fibers in the aorta. The open question was what sets off the turn from falling to rising, and whether menopause is the switch.

What the data show

The American Heart Association’s release on the study describes 6,760 participants, women and men combined, from the Framingham Heart Study, seen at three visits spanning 14 years. Women were sorted into four groups: those still having periods, and those whose menopause came early (before age 48), at an average age (48 to 53), or late (after 53).

After adjusting for risk factors that affect blood vessels, pulse pressure bottomed out at age 47 in men. In women it bottomed out at 39, 38, 37, and 37 across the four groups. For women with early, average, and late menopause, that low point came an average of 6, 14, and 19 years before their final period. When menopause happened had no influence on when the turn came. After midlife, pulse pressure rose with age in both women and men, and after age 60 women’s average was higher.

This study takes apart a tidy story. Estrogen falls, arteries stiffen, pulse pressure climbs: it sounds right. But if the turn comes in the late 30s whether a woman’s periods stop early or late, menopause cannot be what flips the switch. The authors conclude that factors other than the age of menopause are involved, and the structure of the aorta itself may be part of the answer.

The practical lesson is to look at both numbers. Pulse pressure is not part of current clinical guidelines for managing blood pressure. I think it deserves far more attention than it gets, starting well before menopause.

How strong is the evidence?

The Framingham Heart Study is an ongoing, long-term community study that has followed up to three generations of families in Massachusetts. Still, this was an observational study. It shows when pulse pressure changes, but it cannot prove what causes the change. Menopause age was self-reported, and estrogen levels were not measured, so the link to hormones was judged by timing alone. Most participants were of white European descent, so the results may not apply to people from other racial or ethnic groups.

A wider pulse pressure means the heart is working harder, and it can damage the small blood vessels in organs such as the brain and kidneys. Wide pulse pressure is an independent risk factor for heart and blood vessel disease, dementia, and kidney disease, meaning it adds risk on its own. It may also make high blood pressure harder to treat, because blood pressure with a wide gap is less likely to respond to the usual medicines. That is why the study’s senior author says tailored treatment matters for these patients.

Does menopause still matter for women’s heart health?

Yes. This study shows the turn in pulse pressure starts well before menopause, not that menopause has no effect. Samar El Khoudary of Virginia Commonwealth University, who was not involved in the study, noted that vascular aging may begin years before menopause but may speed up as women enter perimenopause. Her advice was not to wait until menopause to start thinking about heart health. By the time of the final period, changes in the blood vessels may already have been underway for years.

Does early menopause change when pulse pressure starts rising?

Not in this study. Women in the early menopause group reached menopause at an average age of 44, and women in the late group at 56, yet their pulse pressure bottomed out at 38 and 37. That is a 12-year spread in menopause age with almost no shift in the turning point. Women who were still having periods hit their low point at 39, which fits the same late-30s pattern. The analysis left out women whose menopause was brought on by surgery or medication, so it does not answer the question for them.

Bottom Line

The timing of menopause does not appear to set off the midlife rise in women’s pulse pressure. In this Framingham study, the turn from falling to rising came in the late 30s, up to two decades before the final period and about 10 years earlier than in men. After age 60, women’s average pulse pressure was higher than men’s.
