Does obesity age the brain faster in men than women?

Yes. In 35,092 UK Biobank adults, staying obese for 16 years raised the odds of an older-looking brain by about 90% in men (odds ratio 1.90) but only about 25% in women (odds ratio 1.25). The same body weight pattern hit male brains far harder.

Brain age is a number you can measure. Researchers feed MRI scans into a computer model that has learned what brains look like at each chronological age. The model then guesses how old your brain looks. If that guess comes in higher than your birth certificate, your brain shows what scientists call brain age acceleration. In this study, 8,198 men (49.73%) and 9,089 women (48.84%) fell into that accelerated group, so roughly half of each sex.

The direction and the sex difference are the useful findings, not the exact size.

What the data show

The research team tracked body measurements over 16 years and sorted people into obesity trajectories, meaning the path their body size took across that time rather than a single snapshot. People who stayed heavy the whole time, the high-stable group, carried the extra brain aging risk. In men that meant about 90% higher odds, very likely between 64% and 121% higher (95% CI 1.64 to 2.21). In women it meant about 25% higher odds, very likely between 12% and 40% higher (95% CI 1.12 to 1.40).

Genes mattered too, and here the sexes looked alike. The team built a polygenic risk score from 55 independent genetic locations, which is a way of adding up many small inherited effects into one number. High genetic risk raised the odds of accelerated brain aging by about 39% in men (odds ratio 1.39) and about 34% in women (odds ratio 1.34).

Stacking the two together did the most damage. Men with both high genetic risk and 16 years of stable obesity had roughly 178% higher odds of an aging brain (odds ratio 2.78), close to triple. Women in that same double-hit group had about 57% higher odds (odds ratio 1.57).

Can losing the weight in midlife help?

The most useful number in this paper is the one that moves. People who shifted from obesity to non-obesity during the follow-up had 30% lower odds of accelerated brain aging in men and 18% lower in women. That reduction showed up even in people carrying high genetic risk, which is the part worth sitting with. A polygenic score is fixed at conception. Body weight is not.

This does not prove that losing weight repaired anyone’s brain. This is observational work, so the people who lost weight may differ from those who did not in ways the researchers could not fully account for, including illness, activity, and income. What the data show is that the group that came out of obesity looked better on brain scans than the group that stayed in it.

The sex difference is the headline, and it is a large one. A 90% versus 25% gap in odds is not a rounding error, and it fits a pattern showing up across cardiometabolic research: the same risk factor does not cost men and women the same amount. Body fat distribution, sex hormones, and inflammation all differ between the sexes, and any of those could explain part of this. The study measured the gap, it did not explain it.

What I take from this practically is that a single weight reading tells you less than a decade of readings. A person who has carried obesity since their forties is in a different situation than someone who gained the same weight last year, and this study is one of the few that can separate those two people.

The genetic finding deserves less weight than it will get. A 39% bump in odds from a 55-locus score is real but modest, and nobody should be ordering a brain aging gene panel. The actionable signal is the trajectory, not the genotype.

How strong is the evidence?

The sample size is a real strength. More than 35,000 people with both repeated body measurements and multimodal MRI is an unusual dataset, and the researchers built separate brain age models for men and women rather than assuming one model fits both. That matters, because a shared model can hide exactly the kind of sex difference this study was looking for.

The limits are the ones that come with UK Biobank. Participants skew healthier, whiter, and better off than the general UK population, so the numbers may not transfer cleanly to other groups. Brain age acceleration is also a research measure, not a diagnosis. A brain that scans older than its years is a risk marker, not dementia, and this study did not follow anyone forward to see who developed cognitive problems.
