Can a daily coffee habit change your hormones and body fat?
Yes, at least on paper. In 2,264 middle-aged Finnish adults, men who drank more coffee had higher total testosterone, and each additional cup per day was linked to about 0.29 nmol/L more. Heavier coffee drinkers of both sexes also carried less body fat and more muscle, even though their BMI looked the same.
The study comes from the Northern Finland Birth Cohort 1966, a group of people followed since birth who came back for a clinical exam at age 46. Finland drinks more coffee than any country in the world, so this is a good place to study heavy, ordinary, everyday coffee use rather than the occasional cup. Researchers at the University of Oulu compared how much coffee people drank with their body composition, blood sugar handling, blood fats, and sex hormones.
The setup matters. Participants fasted for 12 hours overnight and were told to skip smoking and coffee before the blood draw. So these are not the short-term jolt effects of a fresh espresso. They reflect the state of the body in people whose long-running habit is heavy coffee, measured hours after their last cup.
What the data show
Coffee drinkers were sorted into non-consumers, low drinkers at 1 to 2 cups a day, moderate at 3 to 4 cups, and high at 5 or more. Only 70 people drank no coffee at all, while 1,076 were in the high group. Almost everyone drank filtered coffee.
Body weight told one story and body composition told another. Average BMI was nearly identical across the groups, at about 27, and the small gaps between them sat well inside the range you would expect from chance alone. Yet the heavier coffee drinkers had lower body fat mass, lower visceral fat, which is the deep fat packed around your organs, and more skeletal muscle. Testosterone swings a lot within the same man from day to day and hour to hour, so no individual is going to drink four cups and watch a lab value climb in a predictable way.
What makes the paper worth reading is the internal consistency. Higher total testosterone, higher SHBG, less visceral fat, lower branched-chain amino acids, and better insulin numbers in men all point the same direction. Low testosterone and low SHBG in middle-aged men are established markers of metabolic syndrome and diabetes risk, so the hormone findings and the metabolic findings agree with each other instead of contradicting. That coherence is more persuasive to me than any single number in the table.
One finding cuts the other way. Systolic blood pressure was highest in the heaviest drinkers, running about 1.6 mmHg above moderate drinkers and about 3.6 mmHg above non-drinkers. Small, but real, and measured after an overnight fast with no coffee, so it is not just the acute caffeine kick.
How strong is the evidence?
The strengths are real. This is a large, well-characterized birth cohort that captured nearly every birth in Northern Finland in 1966, with standardized fasting protocols and laboratory testosterone measured by mass spectrometry rather than a cheaper immunoassay. Everyone was the same age and broadly the same background, which strips out a lot of confounding.
The weaknesses are equally real. Coffee intake was self-reported, and the questionnaire never defined the size of a cup, so one person’s five cups could be another’s three.
