Just 5 Weeks of Brain Training May Protect Against Dementia

Can a Few Weeks of Brain Training Really Protect Against Dementia?

With a caveat. A 20-year NIH-funded follow-up found that older adults who completed five to six weeks of “speed of processing” brain training and at least one booster session had a 25% lower risk of a dementia diagnosis compared to those who received no training. Speed-trained participants who skipped the boosters showed no reduction in risk. It was the only type of cognitive training in the study to show a lasting protective effect.

The study followed over 2,000 adults aged 65 and older from the ACTIVE trial (Advanced Cognitive Training for Independent and Vital Elderly), a four-arm randomized controlled trial in a diverse sample of 2,802 people recruited across six metropolitan areas in 1998 and 1999. Researchers tracked dementia diagnoses through Medicare claims from 1999 to 2019. The findings were published February 2026 in Alzheimer’s & Dementia: Translational Research and Clinical Interventions.

What the data show:

Dr. Kumar’s Take

This is one of the most interesting aging studies I have seen in years. A non-drug intervention delivered over five to six weeks, with follow-up sessions, and dementia risk tracked for 20 years through Medicare claims. What makes this study especially strong is that it started as a randomized clinical trial, the gold standard of medical research, and it followed participants for two decades.

The headline needs one important qualifier. The 25% reduction applies to people who completed the initial training and at least one booster session at 11 or 35 months. Speed-trained participants who skipped the boosters showed no benefit. So this is not a five-week intervention with a 20-year payoff, it is a five-week intervention plus maintenance.

I also want to be honest about the other limits. We still do not know exactly why speed training worked and the other two types did not. The booster sessions were a second randomization built into the trial design, which is a real strength, though the headline figure describes those who actually completed a booster. And we need more research on whether this approach helps people of different backgrounds equally. But as a practical, low-risk strategy for brain health, this deserves serious attention.

Study Snapshot

Starting in 1998, the ACTIVE trial enrolled 2,802 adults aged 65 and older, of whom 2,021 were in traditional Medicare at baseline and form the sample for this dementia analysis. They randomly assigned participants to one of four groups: memory training, reasoning training, speed of processing training, or no training (control group).

Each training group completed up to 10 sessions over five to six weeks. About half of the participants in each training group also received up to four additional booster sessions at 11 and 35 months after the initial program.

About three fourths of participants were women. Seventy percent were white. The average starting age was 74. Over the 20-year follow-up, roughly three fourths of participants died at an average age of 84.

Results in Real Numbers

Among those who completed speed training with booster sessions, 105 out of 264 (39.8%) were diagnosed with dementia. In the control group, 239 out of 491 (48.7%) developed dementia. Adjusted for baseline characteristics, that works out to a hazard ratio of 0.75 (95% CI 0.59 to 0.95), the 25% lower risk in the headline.

The booster sessions are not a footnote here. Speed-trained participants who did no booster training had a hazard ratio of 1.01 (95% CI 0.81 to 1.27), meaning no reduction in dementia risk at all. The benefit in this analysis belongs to the speed-plus-booster group, not to the initial five to six weeks of training on its own.

Neither memory training nor reasoning training showed a statistically significant main effect on dementia risk at the 20-year mark. Speed training was the only one that stood apart.

How Speed Training Works

Speed of processing training teaches people to quickly spot visual details on a computer screen. The tasks get harder over time. You might need to identify objects that flash briefly or track multiple items at once.

What made this training different from the other two types is that it was adaptive. The difficulty adjusted based on how well each person performed that day. If you did well, the tasks got harder. If you struggled, the pace slowed down.

The other key difference is the type of learning involved. Speed training uses implicit learning, which is more like building a habit or muscle memory. Memory and reasoning training rely on explicit learning, where you consciously study facts and strategies. These two types of learning use different brain systems, which may explain why only speed training protected against dementia.
