Is High LDL Really the Culprit? (Cardiovascular Disease Part 3)

Welcome to Part 3 of our deep dive into cardiovascular disease on The Dr Kumar Discovery Podcast.

Episode Highlights

  • How the Framingham Heart Study shaped the cholesterol narrative
  • What traditional cultures reveal about “normal” LDL levels
  • Why very low LDL is consistently associated with higher mortality
  • What CAC scans show about high LDL and actual disease risk
  • How the Injury Response Hypothesis reframes the role of LDL
  • When statins may help — and when they may not
  • What real-world, unbiased data suggests about cholesterol’s true role

Show Notes

In this episode, Dr. Kumar tackles one of the most debated questions in medicine: Does high LDL cholesterol really cause heart disease?

He walks through the historical roots of the cholesterol hypothesis, beginning with the death of President Franklin D. Roosevelt and the launch of the Framingham Heart Study. From there, he explores LDL levels in traditional cultures, modern epidemiological data, and new imaging tools like the coronary artery calcium (CAC) scan.

You’ll discover:

  • That very low LDL is linked to higher risk of death — especially in older adults
  • Why people with LDL over 190 mg/dL but a CAC score of zero may have little to no heart disease
  • That LDL may be part of the healing response — not necessarily the cause of damage
  • How oxidation, inflammation, and diet — particularly linoleic acid from seed oils — are likely the real culprits

Dr. Kumar introduces the Injury Response Hypothesis, a model that sees LDL as a responder to vascular injury rather than a primary offender. He also gives practical, evidence-based suggestions for how to assess your true cardiovascular risk — including when it makes sense to get on a statin, and when a CAC scan might be all the reassurance you need.

If you’re currently on a statin, being told to start one, or just want a clear, common-sense breakdown of the science, this is the episode for you.
